Patient Information
ATTENTION  PATIENTS: 
When arriving to your appointment, please make sure to have all forms of identification needed for your appointment (which include a state issued photo ID and health insurance cards) If your current insurance plan has changed, please make sure that you have contacted the insurance company to verify that we are listed as your PCP (Primary care Physician) - As this will insure proper assignments of benefits and avoid any billing discrepancies. If experiencing cold/flu like symptoms, please wear a mask as a courtesy to others. 
- ALL Co-Pays and Balances are due at the time of visit
We accept all major credit cards 

A list of all your medications will be needed for review at every appointment - If you recently went to an urgent care, out patient facility, ER or had been admitted to a hospital, please bring ALL discharge paperwork to your next appointment, this includes any and all medications that were given/prescribed to you during this time. 

- All Diabetics must bring their Blood Sugar Logs to EVERY appointment and present them to the medical receptionist upon your arrival to be scanned into your chart - If you keep track of your Blood pressures, make sure to include a list of this log as well.

** Please plan to arrive at least 10-15 minutes prior to your scheduled appointment time.
** If you need to cancel or reschedule your appointment, we do require at least 24 hour notice. This is important to adhere to in order to avoid a fee of $25-$50 
This also gives appropriate time allotted for others to come in to be seen.
** Please be aware that If you are greater than 15 minutes late for your appointment you may be asked to reschedule your appointment.
** New patients must adhere to this policy especially on your first appointment as we may be unable to reschedule you.
 

** Video taping is strictly prohibited in our office 

** When prescription refills are needed, Please contact your Pharmacy directly or contact us using your patient portal. - We ask that you do NOT wait until your medication has been depleted before attempting to get it refilled. Call at least 48 hours prior to and keep in mind that we are closed on weekends & certain Holidays. Also please be aware that it may take a day to get medications filled for you to pick up at your pharmacy. *Non compliant* patients will not be able to get scripts refilled until seen in the office and up to date with any needed testing and appointments. This is highly imperative in order to work together as a team to meet your health care goals and needs.
NEW PATIENTS:
 
Please make sure to review this website and save all needed numbers to contact us. Activate your patient portal as this will be an important tool to utilize for communicating with your Medical Healthcare Team. Highlighted in blue you will find all forms needed to be filled out in their entirety before attending your first initial appointment. if you are unable to access them from this website, please contact Lisa, and she will mail a New Patient packet out for you to fill out prior to your arrival. If there is not enough time in between the scheduled appointment, then arrive to this appointment 15 to 20 minutes prior in order to complete the New Patient forms. You will need your last medical providers name, address, telephone and fax numbers in order to send a records release. A state issued identification card and current updated insurance card with Dr. Ghous Yasin listed as your PCP will need to be present as well. If an updated card with the Drs name is not yet generated, please collect a reference number from your insurance customer service representative. We will need that number to record onto your chart.
